Didn't go on a day? Write "didn't go" so it isn't mistaken for a forgotten day. Free diary from stoollog.com. This is a diary, not medical advice.

## How to fill it in

Write a line right after you go, with the time and the type from the [Bristol stool chart](https://stoollog.com/bristol-stool-chart). Add pain from 0 to 10, and anything unusual: blood, mucus, a colour that isn't brown, straining, or needing to go urgently. In the last column, note foods you suspect, water, medicines with their dose, and symptoms such as bloating or cramps.

Two weeks gives a doctor a good picture, so print two copies. At the end of each week, count your stools, how many were hard (types 1–2) and loose (types 6–7), and the longest gap. Our guide on [how to keep a stool diary](https://stoollog.com/blog/how-to-keep-a-stool-diary) explains what doctors look for, and [what to bring to a GI appointment](https://stoollog.com/blog/what-to-bring-to-a-gi-appointment) has questions to ask.

For a child, use the same sheet: note accidents or soiling in the last column. See [keeping a poop diary for your child](https://stoollog.com/blog/poop-diary-for-kids).

## Rather keep it on your phone?
